Yup. I've focused on using Sitemesh for application development, not wrapping existing code.
If you're using Apache 2 and are interested in Sitemesh-like capability for wrapping legacy data (e.g. raw HTML) or (I think) stuff like PHP, Perl, etc. you might want to check out the Apache 2 filter chain stuff.
